I have genesis rom file "Sonic & Knuckles (World).md". use this command to run:
"mame64" genesis -cart "Sonic & Knuckles (World).md"
the result is 0004.png. but I use "Kega Fusion v3.64 (NOMAPLE) (Windows) (C) Steve Snake 2010" to load the same file ,it runs well.
I also have sk.zip,which includes mpr-16910-u.ic1and mpr-16911-s.ic2. I use this command to run
"mame64" genesis sk
It runs well. screenshoot is 0005.png.

I use the real lock-on rom file "Sonic & Knuckles + Sonic The Hedgehog (World).md",
"mame64" genesis -cart "Sonic & Knuckles + Sonic The Hedgehog (World).md"
the screen is 0003.png. that is right.

I'm not sure if this is a bug.

That's correct. MAME emulates the lock-on hardware and won't work with hacked lock-on images made for other emulators.
