How to create a blank HD image for Apple 2GS ?

Hi,
I´m testing in current Mame the driver apple2gs with a cffa2 card in one slot.
With this card i can connect a CHD hard disk image to the apple2gs, however i don´t know what C,H,S use with chdman.
Since this hard disk must be prodos it should be 30 Mb, right ?
Which option is more compatible for this driver? cffa2 card or Apple II SCSI Card (-scsi)
